2024/25 Home Education Program Participation Agreement
For the QAS Flex program
This agreement provides information pertaining to the responsibilities of parents/guardians while their students are enrolled with Queen of All Saints Academy (“QAS”, “QAS FLEX” or “we” or “our”).
As the parent/guardian of a student registering with QAS, I understand and agree to the following:
Financial Responsibility
- I am a financially responsible party for each student I enroll and agree to pay the tuition. I understand that I may pay the tuition in full at the time of registration or in a monthly payment plan for up to ten months.
- I am aware that QAS may change policies and procedures as needed.
Payment Plans
- Families who elect to send their children to QAS enter into a contractual agreement with the school; therefore they are obligated to pay full tuition as contracted either in a ten-month payment plan or full tuition payment, non-refundable after the first 30 days of enrollment. While there is no interest charged for the monthly payment plan, there is a $25 payment plan fee that is charged with the first payment.
- QAS offers families a risk-free 30-day trial of its program. If I choose to withdraw my student before the 2nd month starts, QAS will drop the course(s) and issue a full refund of the tuition.
- Parents choosing to pay using the Tuition Payment Plan (TPP) can opt into a payment plan for up to 10 installments for the year. If a student completes a semester/year early and wishes to begin the next semester/year at that time, parents using the TTP must pay-off their previous semester/year obligation before their child can proceed.
- I understand that if a monthly payment is returned as non-sufficient funds, and I do not contact QAS to make other financial arrangements, my student will be deactivated from the curriculum and will not have access to the content until my financial obligations have been satisfied.
- I understand that failure to make timely and complete payments may result in my account being turned over to a collection agency for collection of any outstanding tuition charges.
Withdrawal, Cancellation, and Refund Policy
I acknowledge having entered into a Financial Responsibility Agreement with QAS promising to pay full tuition as contracted either in a monthly payment plan or full tuition payment, non-refundable after 30 days. Should I withdraw my student at any time after the first 30 days during the school year, I remain responsible for the full amount of tuition and fees, subject to QAS’s agreement to release me from my financial obligations, as determined by QAS in its sole discretion. The family is also charged a $50 withdrawal fee. Registration ($25 fee for payment plan and/or convenience fees for credit card use), course-related fees (basic school supplies that may have been purchased, e.g., a calculator), and ipad/book purchases are non-refundable.
30 Days Risk-Free
- QAS offers families a risk-free 30-day trial of its program. If I choose to withdraw my student before the 2nd month starts, QAS will drop the course(s) and issue a full refund of the tuition.
- Payment plan fees ($25.00), or any course-related ipad/book purchases are not refundable.
- If I withdraw my student any time after the 30-day period, I will be charged tuition as provided above and in the Financial Responsibility Agreement.
Cancellation of Enrollment
- QAS reserves the right to cancel a student’s enrollment at any time for non-payment of fees, unsatisfactory progress, failure to submit work on a regular basis, or obvious academic dishonesty.
- Once a student is dismissed, reinstatement is considered only upon written application to and approval by the administration of QAS. If reinstatement during the academic year is allowed, tuition fees must be brought current. Reinstated students are automatically placed on probation. The administration will consult with the student’s parents as to the nature and length of the probation.
- If a student’s enrollment is suspended or canceled for any reason, schoolwork that is completed after that date is considered as home-school status and will not be included in your child’s transcript from QAS. Upon reinstatement, if the parent seeks to have the home-school coursework receive credit, all accounts must be brought current, the student must be reinstated, and the coursework tests must be graded by Academy teachers.
- Applications for reinstatement will not be accepted beyond the student’s original course completion deadline. Applicants seeking reinstatement beyond the course completion deadline are considered as having new student status. Depending on the length of time that has passed since enrollment in QAS, the administration reserves the right to ask the student to take a new entrance assessment exam.
- I have read and understood the cancellation and refund policy above. If I choose to cancel or withdraw my student, I must notify QAS in writing or by email.
Financial Contract
FINANCIAL CONTRACT
This Financial Contract constitutes a binding contract between the undersigned parent/guardian (the “Financially Responsible Party” or “I”) and Queen of All Saints Academy.
PAYMENT OF FEES/PROMISE TO PAY
Payment Obligation: I acknowledge that Student is under my legal supervision and expressly acknowledge and agree that I am the Financially Responsible Party under this Agreement. I understand and agree that when I register the Student for any class or receive any services from QAS for Student, I accept full responsibility to pay all tuition, fees, and other associated costs assessed as a result of Student’s registration and/or receipt of services in accordance with the applicable fee schedule according to the “Payment Plan Selection” I previously chose.
Promissory Note Agreement: I further understand and agree that my acceptance of these terms constitutes a promissory note agreement; i.e., a financial obligation in the form of an educational loan as defined by the U.S. Bankruptcy Code at 11 U.S.C. §523(a)(8), in which Queen of All Saints Academy is providing me educational services and deferring some or all of my payment obligation for those services. I promise to pay for all assessed tuition, fees, and other associated costs for this term by the published or assigned due date as reflected in the Payment Plan Selection I chose.
Fee: Queen of All Saints Academy will charge a $25 non-refundable fee for participating in a payment plan; this fee is non-refundable even if a credit is later generated on my account.
Withdrawal: I understand and agree that if Student drops or withdraws from some or all of the classes for which Student is registered after 30 days of the program, I will be responsible for paying all or a portion of tuition and fees in accordance with the policy incorporated herein by reference. I further understand that Student’s failure to attend class or receive a bill does not absolve me of my financial responsibility as described above. I understand that I must comply with all Queen of All Saints Academy procedures and policies regarding Student’s withdrawal to be eligible for a refund of any tuition, costs, or fees (excluding any non-refundable fees designated as such). If Student fails to attend courses, fails to complete required course work, withdraws from Queen of All Saints Academy, withdraws from a course(s), or drops a course(s), I am still liable for the full amount owed. I must notify Queen of All Saints Academy administration no less than one week prior to the course start date of any term in writing informing Queen of All Saints Academy that Student will not be attending to be eligible for any refund. (Enrollment/Admissions Deposit, Lab fees, and Technology fees are non- refundable).
METHOD OF BILLING
I understand that Queen of All Saints Academy uses electronic billing (e-bill) as its official billing method, and therefore, I am responsible for viewing and paying my account e-bill by the scheduled due date. I further understand that failure to review my e-bill does not constitute a valid reason for not paying my bill on time.
BILLING ERRORS
I understand that administrative, clerical, or technical billing errors do not absolve me of my financial responsibility to pay the correct amount of tuition, fees, and other associated financial obligations assessed as a result of Student’s registration at Queen of All Saints Academy.
PAYMENTS / DIGITAL WALLET
I hereby authorize Queen of All Saints Academy to use the bank account information or credit/debit card account information (collectively, “Payment Information”) that I provide for payment of all tuition and fees assessed to my account under this Agreement as a result of Student’s registration and/or receipt of services from Queen of All Saints Academy. It is my obligation to ensure that all financial estimates are accurate on the projected Summary of Account prior to submitting any Payment Information to Queen of All Saints Academy.
I acknowledge that all Payment Information uploaded to my Account will remain in my Wallet until I remove it. I can remove Payment Information from my Wallet at any time so long as it is not linked to an existing Payment Plan Selection. To remove Payment Information linked to an existing Payment Plan Selection, I must follow the procedures outlined in the “Updating Payment Information” section below.
I represent, warrant, and covenant to Queen of All Saints Academy that I have, and will have at the moment of use, all required permission, consents, and authorizations to use any Payment Information provided to Queen of All Saints Academy, and hereby agree to indemnify, defend, and hold Queen of All Saints Academy, its affiliates, officers, trustees, employees, agents and representatives harmless from and against any losses, costs, damages, or expenses (including reasonable attorneys’ fees and related costs) resulting from any third party claims arising from the unauthorized use of any Payment Information provided to Queen of All Saints Academy.
UPDATING PAYMENT INFORMATION
I am solely responsible for ensuring that all Payment Information I provide to Queen of All Saints Academy for purposes of payment, including those uploaded to my Wallet, is complete, up-to-date, and accurate at all times and that my account has sufficient funds at least two (2) days prior to the scheduled draft date under this Agreement.
ZERO OR CREDITED BALANCE ACCOUNT
To the extent my account following completion of registration and upon execution of this Agreement indicates a zero or credit balance, I understand that all provisions of this Agreement apply to my account, including, but not limited to, penalties and immediate payment obligations for failed payments which may have created the zero or credit balance. I understand and agree that any credit balance is merely a projection, and that changes to my account or subsequent charges could occur affecting the amount and timing of any refund of funds related to my account.
RETURNED PAYMENTS/FAILED PAYMENTS
If a payment I make to my account is returned by a bank or other financial institution for any reason, I agree to repay immediately to Queen of All Saints Academy the original amount of the payment together with a returned payment fee and any finance charges Queen of All Saints Academy may in its sole discretion impose thereon at the rate established in this Agreement. I also understand that Queen of All Saints Academy may take legal action to recover funds I owe to Queen of All Saints Academy. I agree that a returned payment and/or failure to comply with the terms of any Payment Plan Selection or agreement with Queen of All Saints Academy may also result in cancellation of Student’s classes and/or suspension of Student’s eligibility to register for future classes at Queen of All Saints Academy; loss of eligibility for Payment Plans for tuition, costs, and fees in future semesters; and/or administrative withdrawal from some or all registered courses. I agree that multiple returned payments and/or failure to comply with the terms of any Payment Plan Selection or agreement with Queen of All Saints Academy will result in Queen of All Saints Academy requiring future payments be tendered in cash, credit card, cashier’s check, or money order. Queen of All Saints Academy may take legal action to recover funds I owe to Queen of All Saints Academy.
DELINQUENT ACCOUNT/COLLECTION
Hold: I understand and agree that if I fail to pay my account bill or any monies due and owing Queen of All Saints Academy by the scheduled due date, including failure of a scheduled draw under a Payment Plan Selection, Queen of All Saints Academy may place a hold on the Student’s account, preventing the Student from registering for future classes, accessing services and curriculum, requesting transcripts, and/or receiving a diploma.
Late Payment Charge: I understand and agree that if I fail to pay my account bill or any monies due and owing Queen of All Saints Academy by the scheduled due/draft date, Queen of All Saints Academy may in its sole discretion assess a late payment and/or finance charge at the rate of 1% each month (12 % APR) on the past due portion of my account until I pay my past due account in full.
Collection Agency Fees: I understand and accept that if I fail to pay my account bill or any monies due and owing Queen of All Saints Academy by the scheduled due date, and fail to make acceptable payment arrangements to bring my account current, Queen of All Saints Academy may refer my delinquent account to a collection agency. I further understand that I am responsible for paying the collection agency fee which may be based on a percentage at a maximum of fifty percent (50%) of my delinquent account, together with all costs and expenses, including interest and reasonable attorney’s fees, necessary for the collection of my delinquent account. Finally, I understand that Queen of All Saints Academy may report my delinquent account to one or more of the national credit bureaus.
COMMUNICATION
Method of Communication: I acknowledge that Queen of All Saints Academy uses Student’s QAS assigned e-mail account as the official method of communication with me, and that therefore I am responsible for monitoring the e-mail account and reading the e-mails I receive from Queen of All Saints Academy on a timely basis.
Updating Contact Information: I understand and agree that I am responsible for keeping Queen of All Saints Academy records up to date with my current physical addresses, email addresses, and phone numbers. Upon leaving Queen of All Saints Academy for any reason, it is my responsibility to provide Queen of All Saints Academy with updated contact information for purposes of continued communication regarding any amounts that remain due and owing to Queen of All Saints.
Contact Information Consent: To the extent that Queen of All Saints Academy has the right to disclose my address, telephone number and email address under federal law, I consent to Queen of All Saints making such disclosures under Florida law so long as such disclosures are either made for non-commercial purposes or limited to school officials, other students, or other members of the Queen of All Saints Academy community. This includes consent to uses within the Queen of All Saints community and to uses in Queen of All Saints processes and operations, such as: to facilitate group projects, class discussions, and course-related activities; and within student clubs, organizations, and activities.
